Forms for requesting Vice Chancellor approval for Facilities Work.

  1. Request for Vice Chancellor Approval for P/T Contracts/Amendments and Purchases over $100,000 or continue beyond 5 years or Sole Source
  2. Request for Vice Chancellor Approval for Construction Contracts/Change Orders over $250,000 or continue beyond 5 years or Sole Source
  3. Request for Vice Chancellor Approval for Emergency Authorization 

Note:  Request for delegation for Capital (General Obligation), Revenue and HEAPR funded projects (Procedure 6.5.5, Part 2), shall continue to be submitted in letter format. Contact your Program Manager for assistance if needed.

HEAPR Funds Transfer Request Form  Rev. 12/12/2012
Use this form when the need arises to request a transfer of HEAPR funds from one project cost center to another.   This form can also be used to request additional HEAPR funds when appropriate. Please contact your PM if you have questions about this form.
